1. Introduction
This Privacy Policy describes how Psychoéducation MT, whose clinic is located at 4826 Saint-Jean Boulevard, Pierrefonds, Quebec H9H 4B2 (“we”, “our” or “us”), collects, uses, retains, discloses and protects the personal information of users (“you”, “your”) in connection with the use of our website available at https://psychoeducationmt.com (the “Site”) and our services. This policy is drafted in accordance with Quebec’s Act respecting the protection of personal information in the private sector (Law 25).
2. Definitions
Personal information: any information concerning a natural person that allows that person to be identified, directly or indirectly, including their name, address, email address, telephone number and any browsing data associated with them. Information about your health and your psychoeducational follow-up is sensitive personal information.
3. Personal information collected
We may collect the following categories of information:
- contact details: name, address, email address, telephone number, date of birth;
- information needed to book an appointment online (requested service, date and time, reason for consultation, where applicable);
- information collected in the course of our professional services (assessment, follow-up, file notes), which is kept in the client file;
- information you voluntarily provide through a form (information or appointment request) or when communicating with us;
- browsing data: IP address, browser type, pages viewed, visit duration, collected in particular through cookies and analytics tools;
- marketing communication preferences, where applicable.
4. Purposes of collection
The personal information collected is used for the following purposes:
- to provide, operate and improve our products and services;
- to process your information requests and to book or manage your appointments;
- to create and maintain your client file in accordance with applicable professional obligations;
- to communicate with you and respond to your requests;
- to send you promotional communications, if you have consented to receive them;

- online appointment booking tool and remote consultation platform (Zoom), where applicable;

The information in your file is protected by the professional secrecy that binds psychoeducators who are members of the Ordre des psychoéducateurs et psychoéducatrices du Québec; it is disclosed only with your consent or where required by law.
These third parties are contractually required to protect your personal information and to use it only for the purposes for which it was disclosed to them.
7. Transfer of personal information outside Quebec
Some of the providers and tools we use, including our Google tools (Analytics, Ads) and possibly our cloud hosting provider, may store or process personal information outside Quebec, including elsewhere in Canada and in the United States. Before making such a transfer, we conduct a privacy impact assessment to ensure that the information receives adequate protection.
8. Retention and destruction of information
We retain your personal information only for as long as necessary to fulfill the purposes for which it was collected, or to meet our legal, accounting or tax obligations. At the end of that period, your information is securely destroyed, anonymized or de-identified.
9. Security and confidentiality incidents
We implement reasonable administrative, technical and physical security measures to protect your personal information against loss, theft and any unauthorized access, disclosure, copying, use or modification.
We keep a register of confidentiality incidents. In the event of an incident presenting a risk of serious injury, we will notify the affected individuals and the Commission d’accès à l’information du Québec, in accordance with our legal obligations.

- the right to have inaccurate information corrected;

- the right to portability, i.e. to receive a copy of your information in a structured, commonly used technological format.
